Permission and Release Agreement:
I hereby authorize/consent myself and/or my child to participate in the above-named Kidcreate Studio activities. I understand and agree that in participating in any art class, there is a possibility of physical injury. I agree to release and hold harmless Kidcreate Studio, including its teachers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold Kidcreate Studio liable for any personal injury or any personal property damage that may occur on the premises before, during or after classes. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights.
I also authorize/consent Kidcreate Dana Point to use any photos, images or likenesses taken of myself and/or my child while at a Kidcreate Studio or at a Kidcreate class for presentation under any legal condition, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content. I understand that there shall be no payment for this release. I understand that I may revoke this authorization at any time by notifying Kidcreate Studio in writing.
